Help us with an awesome customer satisfaction checklist that will ensure customers leave 5 star reviews on Yelp.

We would like a customer satisfaction survey designed to do two things:
1. Ensure that the customer is happy with our work
2. Encourage the customer to write 5 star Yelp reviews.
We have no problems getting lots of great reviews (http://www.yelp.com/biz/ej-plumbing…ntain-view) but it's a challenge trying to stop the posting of 1-4 star reviews after we leave the customer site. They might have an issue with the price, delivery, etc. that we are not aware of until they post a negative review. We want to be proactive and try to minimize this by having them check off a 5 star checklist that essentially has them agree that we provided them with a 5 star service experience.
I have sketched something out already so that you have a clear idea of what we are trying to accomplish. The bottom half of the flyer is for our technicians to keep a record of the flyer that we will leave behind.
We are fine with making this look as "Yelpy" as possible. We're not interested in the customer submitting reviews on any other platform at the moment. Even something this dominant in Yelp-like colors and fonts is fine: https://s-media-cache-ak0.pinimg.co…7ee8d9.jpg
We want the document to be easy to read and we'd like to emphasize the text as we have demonstrated in our comps.
We are open to any fonts or colors. I only used the blue in our comp since it's our company color but we don't mind a radical departure that is easy to read and leaves a good impression with the customer.
